A scan should provide answers, not add to the stress of an already painful joint. An open MRI gives many people a more comfortable way to investigate persistent knee, shoulder, neck, ankle or lower back symptoms, particularly when a conventional enclosed scanner feels difficult because of claustrophobia, restricted movement or pain when lying flat.
For patients trying to understand why their knee catches, their shoulder is weak, or their back pain has not settled, the value of MRI is in the detail it can reveal. It allows a specialist to look beyond what can be seen on an X-ray, helping to shape a treatment plan based on the actual source of the problem rather than assumptions.
What is an open MRI?
An MRI scan uses a powerful magnetic field and radio waves to create detailed images of the body’s soft tissues and bones. Unlike X-rays, it does not use ionising radiation. This makes it particularly useful for examining structures such as cartilage, ligaments, tendons, muscles, discs and nerves.
A traditional MRI scanner is shaped like a tunnel. An open MRI has a more open design around the patient. At Droitwich Knee Clinic, upright MRI scanning can also allow selected examinations to be carried out in a position that is more manageable than lying inside a conventional scanner.
The word open can be reassuring, but it does not mean that every scan is the same. Scanner designs, image quality and the most suitable positioning vary. The right choice depends on the part of the body being assessed, the clinical question and what you can comfortably tolerate.
When an open MRI may be a good option
Open MRI is often considered for people who find enclosed spaces distressing, have a larger body shape, cannot comfortably lie flat, or struggle to keep a painful joint still in a conventional scanner. It may also suit patients whose symptoms are affected by posture or weight-bearing, where an upright position can add useful context to the assessment.
For musculoskeletal problems, the scan may help investigate ongoing symptoms after an injury, pain that has not responded to appropriate initial treatment, or uncertainty following an examination and X-ray. Common reasons include:
- suspected meniscal or ligament injury in the knee, including ACL problems
- rotator cuff disease, tendon tears or other causes of shoulder weakness and pain
- arthritis-related joint symptoms where treatment options need clearer planning
- neck or lumbar spine pain with possible disc, nerve or soft-tissue involvement
- ankle pain after sprains, tendon injuries or persistent instability
An MRI is not automatically necessary for every painful joint. Many conditions can be diagnosed through a careful history, physical examination and X-ray, and some scan findings are common even in people without symptoms. A consultant-led assessment helps ensure that imaging is being used to answer a worthwhile question and that the result will genuinely influence your care.
What can an open MRI show?
For knee pain, MRI can show the menisci, cruciate and collateral ligaments, articular cartilage, bone bruising, tendon problems and fluid within the joint. This can be valuable after a twisting injury, when the knee locks or gives way, or when pain persists despite sensible rehabilitation.
For shoulder symptoms, it can assess the rotator cuff tendons, biceps tendon, labrum, joint cartilage and surrounding soft tissues. This is particularly helpful when night pain, weakness or loss of movement raises concern about more than simple inflammation.
In the neck and lower back, MRI can provide information about spinal discs, facet joints, nerves and the spinal canal. The scan is only one part of the picture. Its findings need to match your symptoms, examination and medical history before treatment decisions are made.
MRI can also help with planning when surgery is being considered. If a patient is considering ACL reconstruction, rotator cuff repair or knee replacement, imaging can clarify the condition of key structures and support a more informed conversation about the likely benefits and limits of each option.
Open MRI versus a conventional MRI
The main advantage of an open MRI is comfort and accessibility. Seeing more of the room can make a significant difference for someone with claustrophobia, while a more accommodating position may reduce discomfort for those with severe joint or back pain. Remaining comfortable enough to keep still is important, as movement can blur the images.
However, the best scanner is not always simply the most open one. Conventional high-field scanners can provide very detailed images and may be recommended for certain clinical questions. Open and upright systems have their own strengths, especially where tolerance, positioning and patient access are the limiting issues.
A good service does not treat one type of scanner as the answer for everybody. It considers what needs to be seen, whether the scan position reflects your symptoms, and whether the images will be sufficient for confident clinical decision-making. If another imaging method would be more appropriate, that should be explained clearly.
What happens during the scan?
Before your appointment, you will complete a safety questionnaire. MRI is not suitable for everyone, so it is essential to tell the team about any pacemaker or implanted device, previous surgery, metal fragments, clips, cochlear implants, medication pumps, pregnancy, kidney problems or possible metal in the eyes. Many modern implants are MRI-compatible, but this must always be checked in advance.
You will usually be asked to remove jewellery, watches, hearing aids, bank cards and clothing with metal fastenings. Depending on the body part being scanned, you may lie or sit in a supported position while the radiographer carefully aligns the area of interest.
The scanner makes repetitive tapping and knocking sounds while images are taken. Ear protection is provided. The important practical task is to remain as still as possible for each sequence, which may last several minutes. The scanning team can communicate with you throughout, and you should tell them if you are becoming uncomfortable.
Most musculoskeletal MRI examinations take between 20 and 45 minutes, although this varies according to the area being assessed and the images required. Contrast dye is not needed for many knee, shoulder, ankle and spine scans, but it may occasionally be recommended where it would provide clinically useful additional information.
Preparing for an open MRI scan
Preparation is usually straightforward. Wear comfortable clothing without metal where possible, bring details of previous operations or implants, and continue your usual medication unless you have been given different instructions. If pain makes a particular position difficult, mention it before the appointment rather than trying to endure it in silence.
If you are anxious about the scan, say so when booking. Knowing what will happen, having time to ask questions and choosing the most suitable scan arrangement can make the experience far more manageable. It is also worth bringing previous imaging reports or relevant medical information if these are available, as comparison can sometimes help interpretation.
Turning scan results into a treatment plan
A scan report is useful, but it is not the same as a diagnosis on its own. The most helpful next step is to review the images alongside your story: where it hurts, what movements provoke symptoms, how the problem began, what treatment you have already tried and what you need to get back to doing.
That may lead to a focused rehabilitation plan, activity modification, medication advice, an image-guided or joint injection, or discussion of surgery where there is a clear reason to consider it. Just as importantly, it may show that surgery is not the best first step. Clear imaging can prevent unnecessary delays, but it can also prevent unnecessary treatment.
