What does an open shoulder MRI scan show?
MRI uses a strong magnetic field and radio waves to create detailed images. Unlike an X-ray, it does not use ionising radiation. While X-rays are excellent for showing bone shape, arthritis and fractures, an MRI is often better at assessing the structures that commonly cause ongoing shoulder pain.
An open shoulder MRI scan may help assess the rotator cuff tendons, biceps tendon, labrum, cartilage, joint lining, ligaments, muscles and the bone beneath the joint surface. It can identify signs of tendon inflammation, partial or full-thickness rotator cuff tears, bursitis, arthritis, frozen shoulder changes, impingement-related problems and certain injuries following a fall or sporting incident.
The scan does not replace a clinical assessment. Imaging findings need to be considered alongside your symptoms, strength, range of movement and how the pain affects your life. For example, a rotator cuff tear on an MRI does not always explain a person’s pain, particularly as some tendon changes become more common with age. Equally, a scan that does not show a major tear can still support treatment for painful inflammation or stiffness.
When is open MRI useful for shoulder pain?
An MRI is not required for every painful shoulder. Many recent strains improve with time, activity modification, pain relief and a sensible rehabilitation programme. However, a scan may be useful when pain has not settled, movement is significantly restricted, weakness is affecting work or daily activities, or the diagnosis remains uncertain after examination and initial treatment.
It is commonly considered when a clinician suspects a rotator cuff tear, particularly after an injury or where there is marked weakness lifting the arm. It can also be useful for persistent pain after physiotherapy, suspected labral injury in active patients, unexplained night pain, recurrent instability or planning treatment such as an injection or surgery.
The right test depends on the question being asked. Musculoskeletal ultrasound can assess tendons dynamically and may be an excellent first investigation in some cases. X-rays remain important where arthritis, bone changes or a fracture are suspected. MRI provides a broader picture of the internal joint and surrounding soft tissues. A specialist should advise which investigation is likely to give the clearest answer, rather than arranging a scan simply because shoulder pain has lasted a certain number of weeks.
What happens during the scan?
Before your appointment, you will complete a safety questionnaire. MRI is not suitable for everyone, so tell the team about any pacemaker, implanted device, metal fragment injury, previous surgery, clips, cochlear implant or pregnancy. Many modern joint replacements and surgical implants can be scanned safely, but this must always be checked in advance.
You will usually be asked to remove jewellery, watches, bank cards and other metal items. For a shoulder scan, you will be positioned carefully so the affected shoulder is centred in the scanner. The team will use supports to help you remain comfortable and still. Keeping still matters because movement can blur the images and may mean sequences need to be repeated.
The scanner makes tapping and knocking noises while it takes images. You will be given hearing protection and can communicate with the radiography team throughout. The exact design of an open MRI varies, but it is generally less enclosed than a traditional tunnel scanner. This can make a real difference for people who are anxious about confined spaces.
Most scans take around 20 to 40 minutes, depending on the images required and whether both shoulders need assessment. MRI contrast is not routinely needed for many shoulder problems. If contrast is recommended, the clinician should explain why it may improve the diagnostic information and discuss any relevant safety considerations.
Is an open MRI as accurate as a conventional MRI?
This is a sensible question, and the answer depends on the scanner, the body part being examined and the clinical detail required. Modern open MRI technology can provide very useful images for many shoulder conditions, particularly when it allows a patient to tolerate the scan comfortably and remain still.
Some conventional closed scanners operate at a higher magnetic field strength and may offer finer detail for certain complex questions. In other situations, the benefits of an open system – greater comfort, easier positioning and less anxiety – may lead to better-quality images than an incomplete or movement-affected scan in a closed scanner.
The key is not to think of open and closed MRI as a simple ranking. The best scan is the one that is safe, appropriate for the clinical question and produces images a specialist can use to guide your care. If there is a concern that a particular finding needs more detailed imaging, your clinician can explain whether another test would add value.
Turning scan results into a treatment plan
A report alone can feel technical and leave patients with more questions than answers. Terms such as tendinopathy, bursitis, degenerative change or partial tear can sound alarming without context. What matters is how those findings relate to your pain, movement, goals and examination.

Does the scan hurt?
MRI itself is painless. Remaining in one position may be uncomfortable if your shoulder is very painful, but the team can use cushions and supports and will guide you through the process. Let them know if you are struggling rather than trying to tolerate significant discomfort.
Can I eat and drink beforehand?
For a standard shoulder MRI, you can usually eat and drink normally unless you have been given different instructions. If contrast or another procedure is planned, follow the appointment guidance provided.
Will an MRI show frozen shoulder?
MRI can show features that support a diagnosis of frozen shoulder and can help rule out other causes of restricted movement. However, frozen shoulder is primarily diagnosed from the pattern of stiffness and the clinical examination, not from imaging alone.
